Change register template (CSV)
A change register template with the exact columns the checker reads and three invented rows. Fill it in, paste it back, and every change comes back with its notification lines and the requirements it touches.
The columns

| Answers | the type's own questions as key=value pairs separated by semicolons: who=ndis; what=serious_injury | for the lines that turn on them |

Your own change log works as it is: the column names a quality system or a spreadsheet exports (Date of change, Category, Description, Date aware, Date notified and the rest) are read without renaming. Never put a resident's, participant's or worker's name in a label.
