    When the provider becomes aware of a death, serious injury, abuse or neglect, unlawful sexual or physical contact or assault, or sexual misconduct including grooming, it must notify the Commissioner within 24 hours with: its name and contact details; a description of the incident; the impact or harm to the person (except for a death); the immediate actions taken to ensure the health, safety and wellbeing of affected persons and whether police or another body was told; the notifier's name and contact details; the time, date and place if known; the names and contact details of those involved; and any other information the Commissioner requires. The notice may be by telephone or in writing on the approved form.

A change register you keep as you go

Notifications to the Aged Care Quality and Safety Commission and the NDIS Quality and Safeguards Commission turn on changes: a new key person, a new site, an incident, a change of ownership. Each has a clause and a period, and each touches requirements an auditor reads later. Recording the change once, with the line and the evidence, turns a registration renewal from a hunt through the year into reading your own register. The one-page sample register shows what twelve entries look like printed.

Scope. Each state or territory authorises regulated restrictive practices under its own scheme; we do not hold the state or territory text, so the checker never says whether a practice is allowed or authorised in a state. Whether an incident is reportable, its priority, and whether an injury is a serious injury are your call: for aged care the Commission's SIRS decision support tool helps decide, and the checker runs the clock and keeps the register once you have.
